Our pathway to the community for individuals in prison and secure services support gentleman in a range of specialist care homes and supported accommodation services who present with Mental Health, Neurological conditions and complex needs.

Our Heron View service has been supporting HMPPS and Justice Teams to transition

prisoners from HM Prisons and Service Users from NHS England secure hospitals nationally since it opened three years ago.

  • Adult males (18+) who have offended or who are at risk of offending
  • Individuals managed by MAPPA, Jigsaw and other agencies
  • Individuals stepping down from HM Prisons and secure hospital settings
  • Aggression & challenging behaviour
  • Sexual disinhibition
  • Risk to themselves and/or others

  1. Dedicated Business Development Manager to support you with your referral from start to finish
  2. Help with the assessment and care planning providing detailed risk assessments if required
  3. Assistance if needed with securing Our pathway to the community for individuals in prison and secure services support funding
  4. Attendance at key meetings
  5. Day visit arrangements
  6. Pathway to supported accommodation if needed

 

Case Studies

CASE STUDY: 1

Mr X came to Heron View in 2020 and prior to his placement with us he was based at HMP Thameside Prison in London. His placement team had identified that Mr X needed a specialist Residential Care Home that could support his ongoing Mental Health needs and Neurocognitive Disorder. When Mr X arrived at Heron View he settled into the service

within his first week and the team had a already set out a robust risk management and care plan that would support him while in the service. Mr X now considers Heron View as his home and enjoys planning and discussing how he intends to live his life in a least restrictive setting with the full support of his dedicated team.

 

CASE STUDY: 2

Mr X now actively engages with the daily activities that take place at the service and in the community such as table tennis, football, bingo, music class and gardening. Mr X is very happy at Heron View and is always thanking staff for supporting him in his new environment and offering him the tools and care to live out a meaningful and happy life.
